Approach to Full Mouth Reconstruction for Extensive Wear A Case Study by Dr. Alla Brown

Case Background

Pat, a 64-year-old patient, was not happy with his smile due to significant wear and tear on his teeth. Over the years, this extensive wear had compromised the appearance and functionality of his smile, diminishing his confidence and comfort. To address this, a full mouth reconstruction was proposed, focusing on both restorative and aesthetic improvements. Given the complexity of the case, a systematic, phased approach was planned to ensure optimal results and patient comfort throughout.

Assessment & Treatment

Pat’s dental examination revealed extensive enamel wear on both the upper and lower teeth, making full occlusal rehabilitation necessary. To restore his smile and function, the treatment plan included the placement of Emax restorations. The process was divided into two main stages to ensure a smooth transition and precise outcomes:

  1. Stage 1: Focused on rebuilding Pat’s upper and lower arches by restoring 10 teeth on both the upper and lower jaws.
  2. Stage 2: Added first molars on both the upper and lower arches to enhance occlusal stability and function.

This approach allowed for a controlled transition to the new occlusal scheme while ensuring a comfortable and natural bite for Pat.

 

 

Preparation and Mock-Up

At the preparation appointment, Pat’s teeth were prepared, and a mock-up was performed to offer a preview of the anticipated final result. This mock-up provided a visual representation, allowing Pat to assess the look and feel of his new smile. Ensuring that Pat felt comfortable and confident with the proposed changes was an essential part of the planning phase.

Restoration Process

Once Pat was comfortable with the mock-up, the first stage of restorations was completed, placing Emax restorations on 10 upper and 10 lower teeth. This phase of treatment was critical in re-establishing his smile’s appearance and restoring core functionality. Following a brief period of adjustment, the second stage was implemented, adding first molars to both arches to complete the occlusal scheme and provide Pat with a balanced, stable bite.

Outcome

The comprehensive full mouth reconstruction gave Pat a smile he could feel confident about. By using Emax restorations, Pat’s teeth now have the durability and aesthetic appeal that both enhance his overall appearance and reduce the risk of future wear.
